Request a Report
Welcome to the request a report or record page for the Tacoma Fire Department. The department has two primary sets of records: fire and code inspections and patient medical records. Please see below for instructions on how to request records.
Request a Patient Record or Medical Report
To request patient records or medical reports, you will need to do the following.
Step 1: If you are the patient:
- You need a signed Patient Request for Access Form.
- Driver’s license/photo identification (a copy is acceptable for faxed or emailed requests).
Step 2: If you are not the patient:
- You need a signed Patient Request for Access Form.
- A copy of the document/s granting legal authority to the representative who signed, for example, a birth certificate, or a designation of legal guardianship, or a medical power of attorney.
- The requestor’s driver’s license/photo identification (a copy is acceptable for faxed or emailed requests).
Step 3: You can submit your request by one of these methods:
- In person at Tacoma Fire Headquarters, 901 Fawcett Avenue, Tacoma, WA 98402
- Via fax at (253) 591-5746
- Via email at HIPAA@tacoma.gov

Fire Investigation Reports or Fire Code Inspection Records
To obtain a fire investigation report or fire code inspection record, please use the City of Tacoma Public Records Center portal link below or email pdr@tacoma.gov.
